Job Summary:We are seeking a detail-oriented Virtual Care Coordinator to support telehealth and remote patient monitoring initiatives within a dynamic healthcare environment. This role involves coordinating virtual care services monitoring patient status remotely and ensuring high-quality patient-centered care through effective communication and collaboration with clinical and technical teams.
Key Responsibilities:
Monitor patients remotely for safety and clinical needs
Support hospital-based telemedicine services and virtual care workflows
Assist in implementing telehealth and remote patient monitoring programs
Collaborate with clinical teams leadership and technical partners
Communicate effectively with patients providers and support teams
Monitor hospital environmental status through virtual systems
Assist in training coordination and telehealth best practice implementation
Maintain accurate documentation in EMR systems
Ensure adherence to quality safety and compliance standards
Required Qualifications:
Minimum 2 years of telehealth experience (required)
Experience in direct patient care
Strong computer skills and familiarity with EMR systems
Excellent organizational and multitasking abilities
Strong attention to detail
Effective verbal and written communication skills
Preferred Skills:
Experience working with telehealth platforms and remote monitoring tools
Ability to collaborate with cross-functional teams including clinical and IT
Strong problem-solving and coordination skills
