Position Summary

Toronto Rehabilitation provides hemodialysis at Bickle Centre site. The unit provides hemodialysis for patients who are post-acute care hospitalization. These patients are attending Low Tolerance and Long Duration (LTLD) Geriatric rehabilitation or Complex Continuing Care. We provide daily and conventional hemodialysis

Duties

• The nurse is to provide safe and competent care through application of nursing knowledge specific to hemodialysis technical aspects of professional practice and demonstrates professionalism and respect
• Responsible for the assessment nursing diagnosis integrated care plan development implementation and evaluation of nursing care.
• Participate in professional program and hospital wide initiatives including self professional development
• Provides leadership supervision and mentorship to new staff when needed
• Compliance with and commitment to patients safety responsibilities and corresponding hospital plans policies and procedures to ensure a safe environment for patients

Qualifications :

• BSCN preferred
• Member in good standing with College of Nurses
• Nephrology Nursing Certification CNeph or willing to complete within 2 years
• Current Hemodialysis Experience preferred
• Member of CANNT
• Excellent patient assessment skills
• Excellent communication interpersonal and leadership skills
• Successful completion of Basic Cardiac Life Support ( C level) Heart Saver Plus (Canada)
• Superior clinical decision-making skills that reflect patient centredness problem solving and critical thinking
• Demonstrates knowledge skill and judgement in the application of hemodialysis
• Demonstrates knowledge and understanding of nursing standards as they apply to the care of patients with chronic kidney disease
• Computer proficiency is required or willing to pursue computer training
• Proficiency with the EPIC electronic health record (EHR) system is an asset.
• Ability to identify and meet own learning needs for pursuing ongoing professional development
