Job Summary

Introduction

Orange Aboriginal Medical Service (OAMS) is growing and so is our Yiriyirimbang Out of Home Care Team. Due to continued growth and increasing demand for our services we are seeking a passionate and dedicated Case Manager to join our expanding team.

This newly created position offers an exciting opportunity to make a meaningful difference in the lives of Aboriginal children young people families and carers while working within a supportive culturally strong and community-focused organisation.



Description

About the Role

As a Yiriyirimbang Case Manager you will provide case management support advocacy and care coordination to children and young people in Out of Home Care (OOHC). Working closely with children families carers communities and partner agencies you will help achieve positive outcomes support permanency planning and ensure children remain connected to their family culture and community.

Key Responsibilities
  • Manage a caseload of children and young people in OOHC.
  • Develop implement and review case plans and cultural support plans.
  • Conduct regular home visits with children young people and carers.
  • Support placement stability through effective case management and carer support.
  • Work collaboratively with families carers DCJ and other service providers.
  • Support restoration and permanency planning where appropriate.
  • Prepare reports case notes and court documentation.
  • Advocate for Aboriginal children young people and families.
  • Maintain accurate and timely records in line with legislative and organisational requirements.
  • Participate in team meetings supervision and professional development opportunities.

About You

To be successful in this role you will have:

  • Certificate IV in Community Services Social Work Welfare or a related field and/or a minimum of two years demonstrated experience in a similar role.
  • Experience in case management and working with vulnerable children young people and families.
  • Strong communication and relationship-building skills.
  • Excellent organisational and time management skills.
  • The ability to work independently and as part of a multidisciplinary team.
  • Strong administrative and record-keeping skills.
  • A commitment to culturally safe child-centred and strengths-based practice.

Mandatory Requirements

  • Current NSW Class C Driver Licence.
  • Current NSW Working With Children Check (WWCC).
  • Current National Criminal History Check.
