Schedule: Monday- Friday 8 am - 4 pm

Brookhaven Health Care Facility is looking to add a Wound Care RN to its clinical this role youll provide specialized wound care services while supporting resident healing infection prevention and overall quality outcomes. RESPONSIBILITIES:

• Provide direct nursing care to residents in accordance with care plans physician orders and wound care protocols
• Assess monitor and document wound status treatment effectiveness and resident progress
• Administer medications IV therapies treatments and wound care interventions as prescribed
• Collaborate with physicians nursing staff therapy teams and other interdisciplinary team members to support resident recovery and care goals
• Develop implement and update wound care plans as needed
• Monitor for signs of infection skin breakdown and changes in resident condition reporting concerns promptly
• Supervise and support CNA and LPN staff to ensure quality resident care
• Maintain accurate documentation assessments care plans and clinical records
• Follow infection control resident safety and HIPAA guidelines
• Perform additional duties as assigned

REQUIREMENTS:

• Valid New York State Registered Nurse (RN) License required
• CPR Certification required or obtained upon hire
• Experience in skilled nursing rehabilitation long-term care acute care or wound care preferred
• Strong clinical assessment wound management and critical thinking skills
• Ability to supervise delegate and collaborate effectively with interdisciplinary teams
• Excellent communication organizational and documentation skills
• Commitment to providing compassionate resident-centered care
