Schedule: Monday- Friday 8 am - 4 pm
Brookhaven Health Care Facility is looking to add a Wound Care RN to its clinical this role youll provide specialized wound care services while supporting resident healing infection prevention and overall quality outcomes. RESPONSIBILITIES:
- Provide direct nursing care to residents in accordance with care plans physician orders and wound care protocols
- Assess monitor and document wound status treatment effectiveness and resident progress
- Administer medications IV therapies treatments and wound care interventions as prescribed
- Collaborate with physicians nursing staff therapy teams and other interdisciplinary team members to support resident recovery and care goals
- Develop implement and update wound care plans as needed
- Monitor for signs of infection skin breakdown and changes in resident condition reporting concerns promptly
- Supervise and support CNA and LPN staff to ensure quality resident care
- Maintain accurate documentation assessments care plans and clinical records
- Follow infection control resident safety and HIPAA guidelines
- Perform additional duties as assigned
REQUIREMENTS:
- Valid New York State Registered Nurse (RN) License required
- CPR Certification required or obtained upon hire
- Experience in skilled nursing rehabilitation long-term care acute care or wound care preferred
- Strong clinical assessment wound management and critical thinking skills
- Ability to supervise delegate and collaborate effectively with interdisciplinary teams
- Excellent communication organizational and documentation skills
