Job Summary

Job Summary
The Workforce Academy Transition Intervention Specialist will use a person-centered approach to support students with disabilities in developing individual transition plans targeting post-secondary transition goals in the areas of education employment and/or independent living. This position requires collaborating with a variety of external business partners and agencies.

Job Duties & Knowledge
Teaching Skills
- Maintain special education documentation and compliance
- Develop and deliver age-appropriate transition goals supporting post-secondary outcomes
- Teach Workforce Academy curriculum
- Implement program components and pacing with fidelity
- Supervise Job Skill Trainers and Paraprofessionals
- Knowledgeable of and experience in teaching students with Multiple Disabilities Autism Significant Cognitive Disabilities and Extensive Support Needs
- Collect and interpret transition data to support individual students post-secondary goals
- Co-Teach with transition intervention specialist
- Development of person-centered transition plans

Innovation
- Experience in developing innovative supports to meet unique student needs
- Knowledge of current best practices in transition planning
- Ability to help students recognize growth in independent living education and employability skills
- Develop and implement high-quality interventions to support student success
- Develop and maintain strong family relationshiops

Community
- Knowledgeable about transition activities and community programming including adult day programs
- Develop and revise Transition Individualized Plans based on individual needs
- Visit Student worksites and provide support as needed
- Knowledgeable of and experienced in providing consultation about transition activities and community worksites to school personnel students and parents
- Help students transition toward independence
- Coordinate transition activities for career and community readiness
- Maintain all special education paperwork
- Coordinate student work rotation meetings
- Support worksite staff working with students in transition to work activities
- Plan and hold Business Advisory Meetings


Relationships
- Establish relationships and networks of support for all families
and students-set up network of agencies based on individual student needs
- Develop and sustain collaborative relationships with outside agencies team members and service providers
- Build long-term relationships with employers and community organizations
- Coordinate and schedule services with internal and external partners in furtherance of students transition goals
- Experience in developing and implementing appropriate goals with parents and students
- Ability to work collaboratively with related service providers to help student progress
- Other duties as assigned by the DSS Director and Manager
