Position Summary

The Supportive Living Coordinator (SLC) provides leadership and oversight to Direct Support Professionals (DSPs) who deliver services to individuals with intellectual and developmental disabilities. This role is responsible for staff supervision scheduling individualized service delivery training compliance and coordination with families and other stakeholders.

The Supportive Living Coordinator plays an important role in ensuring individuals receive consistent high-quality person-centered support while creating a positive and supportive environment for both the individuals we serve and our DSP team.

  • Train supervise and support Direct Support Professionals to ensure high-quality service delivery.
  • Create and maintain weekly staff schedules ensuring all contracted service hours are appropriately covered.
  • Conduct performance evaluations and provide ongoing coaching and constructive feedback to DSPs.
  • Develop and implement Individual Service Plans (ISPs) in partnership with appropriate stakeholders.
  • Monitor and document the effectiveness of ISPs and coordinate adjustments when necessary.
  • Advocate for the rights choices and needs of the individuals we serve in accordance with their ISP and applicable Ohio requirements.
  • Coordinate and facilitate staff training in areas such as communication crisis intervention person-centered care and other required competencies.
  • Promote continuous learning professional development and the effective use of technology and innovative practices.
  • Serve as a liaison among DSPs individuals served families and community resources to support coordinated care and services.
  • Facilitate monthly team meetings as required to promote communication consistency and collaboration.
  • Ensure services comply with Ohio Department of Developmental Disabilities (DODD) Medicaid and Creative Foundations policies and requirements.
  • Stay informed of regulatory and industry changes impacting services for individuals with intellectual and developmental disabilities.
  • Follow established crisis intervention and on-call procedures and provide guidance to DSPs during emergency situations.
  • Provide direct coverage when a scheduled shift is abandoned or cannot otherwise be filled.
  • Maintain accurate and timely documentation related to service delivery incidents staffing and employee performance.
  • Prepare and submit required reports to leadership within established deadlines.
Skills & Qualifications
  • Strong leadership coaching and staff-management skills.
  • Ability to effectively schedule and coordinate staff coverage in a direct-care environment.
  • Strong communication and interpersonal skills with the ability to work effectively with individuals served families DSPs leadership and community partners.
  • Ability to manage multiple priorities and respond effectively to changing staffing and service needs.
  • Strong organizational documentation and follow-through skills.
  • Commitment to person-centered care and supporting individuals with intellectual and developmental disabilities.
  • Ability to respond calmly and appropriately during emergencies or challenging situations.
  • Comfortable using technology and learning new systems and tools.
  • Knowledge of I/DD services Medicaid DODD requirements ISPs and/or direct support services preferred.
Education & Experience
  • High school diploma or GED required; associate or bachelors degree in human services social work psychology healthcare education or a related field preferred.
  • Previous experience supporting individuals with intellectual and developmental disabilities strongly preferred.
  • Previous supervisory team lead case management or program coordination experience preferred.
  • Experience working with Direct Support Professionals or in a community-based service environment is a plus.
Additional Expectations

This position participates in an on-call rotation and may be required to provide direct coverage when a shift cannot otherwise be staffed. Flexibility and a commitment to ensuring continuity of services are essential to success in this role.
