Social Worker | Regional Float

Join the inpatient care management team as a regional float social worker. This unique role will have the ability to support multiple CentraCare hospitals as a float social worker.

Our Socials Workers serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. Youll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary teamof nurses therapists physicians and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.

Schedule

• Full-time 80 hours every two weeks Onsite
• Day shift Monday through Friday 7:30am-4:00pm

Pay & Benefits

• Pay based on experience and degree
  • Bachelors in Social Work starting at $32.09 per hour
  • Masters in Social Work starting at $33.15 per hour
  • Pay range: $30.65 - $49.73 per hour

Qualifications

• Current Licensed Social Worker (LSW) in Minnesota required.
• Bachelors or Masters degree in Social Work required.
• 2 years or more of clinical experience as a Social Worker.
• A valid drivers licensed and a driving record in good standing required.
• American Case Management Association (ACMA) Case Manager-SW Certification required within five years.
