Job Summary

Join our dedicated team at St. Ann Rehabilitation and Nursing Center in Dover NH where youll make a meaningful difference in the lives of those we serve. Our 54-bed skilled nursing facility provides both short-term rehabilitation and long-term care in a warm compassionate environment that focuses on each residents individual needs. We are seeking a full-time Social Worker to join our Social Services this role you will advocate for residents while supporting their emotional psychosocial and medically related needs. Working closely with residents families and the interdisciplinary care team you will help ensure residents receive individualized support their rights are respected and they experience the highest quality of life throughout their stay.

What Youll Do
  • Advocate for residents rights dignity and emotional well-being.
  • Complete psychosocial assessments and develop individualized care plans.
  • Support residents and families through admissions care planning and discharge planning.
  • Coordinate family meetings and collaborate with the interdisciplinary care team.
  • Connect residents with community resources and needed services.
  • Address resident concerns and grievances while promoting person-centered care.
  • Maintain accurate documentation and ensure compliance with state and federal regulations.
What Were Looking For
  • Bachelors degree in Social Work or a related field required.
  • Minimum of 3 years of supervised social work experience in a healthcare setting.
  • Strong communication organization and relationship-building skills.
  • Knowledge of discharge planning advance directives and long-term care regulations preferred.
  • Supervisory experience is a plus.
