This is more than a case management role: its an opportunity to serve as a trusted advocate collaborative leader and resource for both members and colleagues. As a Senior Case Manager youll work with individuals experiencing chronic and acute health conditions helping them overcome barriers to care through personalized support education and coordinated services. Youll build meaningful relationships with members caregivers healthcare providers and community partners to develop solutions that improve health outcomes and enhance quality of life. Every day brings the opportunity to solve complex challenges identify creative resources and ensure members receive the right care at the right time. This role offers a unique combination of direct member engagement and professional leadership. Youll independently manage a diverse caseload while serving as a mentor and resource for fellow case managers helping onboard new team members sharing best practices and contributing to a collaborative high-performing environment. Required Education and Experience:

  • Bachelors degree in Social Work Psychology Sociology or other Health or Human Services related discipline.
  • 24 months experience in home and community-based services case management or related healthcare setting.

Required Certifications and Licenses:

  • Certified Case Manager (CCM).The employee is allowed 1 year from hire date or promotion to Senior Case Manager to obtain the CCM certification.

Department Preferences:

  • One year of paid professional experience in direct service provision care planning and has provided services to the aging and/or disabled population in a home-like setting.
  • Case Management Certification upon hire or post hire.
