What You Will Do:

  • Provide nursing case management services for individuals with behavioral health needs ensuring continuity of care.
  • Conduct health assessments and develop individualized care plans.
  • Collaborate with interdisciplinary team members to coordinate treatment and support services.
  • Monitor patient progress adjust care plans as necessary and ensure documentation is accurate and timely.
  • Educate patients and families about treatment plans medications and self-care strategies.
  • Facilitate referrals and linkages to community resources.

Who You Are:

  • Registered Nurse with 1 years of experience licensed in Massachusetts (required)
  • Strong assessment documentation and communication skills.
  • Reliable transportation and willingness to travel within the service area.
  • Must be at least 21
