What Youll Do

Youll support 610 community homes (about 2535 individuals) by providing routine health oversight:

• Daily wellness checks during home visits
• Monitoring chronic conditions
• Reviewing medications & MARs
• Coaching staff on simple health tasks
• Preparing individuals/staff for medical appointments
• Documenting health updates
• Communicating with families providers and internal teams

This is fieldbased community nursing with a consistent rhythm.

Key Responsibilities

• Health monitoring & earlychange detection
• Medication oversight & reconciliation
• Chronic disease support (diabetes hypertension seizures etc.)
• Appointment coordination & followthrough
• Staff training on practical health skills
• Collaboration with Behavioral Services
• Support during hospital/ER transitions
• Documentation & quality tracking

What You Bring

• Active NJ RN license
• ADN or BSN
• 2 years of nursing experience (community home care residential or IDD a plus)
• Strong assessment & communication skills
• Comfort working independently in the field
• Valid drivers license & reliable transportation
