Resident Care Coordinator (MDS Coordinator) RN or LPN

Citizens Memorial Hospital (CMH) is hiring a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as an MDS Coordinator/Resident Care Coordinator at Ash Grove Healthcare Facility in Ash Grove Missouri someone who leads with skill compassion and a drive to positively impact every resident and family we serve.

What Your Day Looks Like: This skilled nursing facility is a 82-bed facility with two memory care units where you will exercise a number of skills including completing auditing and submitting Medicare Medicaid and PPS assessments (MDS 3.0) according to regulatory timelines. The MDS Coordinator/Resident Care Coordinator is responsible for collaborating with the Interdisciplinary Team (IDT) to develop and update individualized comprehensive care plans reviewing clinical records and nursing documentation and ensuring quality resident outcomes through effective care coordination.

As an MDS Coordinator / Resident Care Coordinator you will be:

Completing auditing and submitting Medicare Medicaid and PPS assessments (MDS 3.0) according to regulatory timelines
Collaborating with the Interdisciplinary Team (IDT) to develop and update individualized comprehensive care plans
Reviewing clinical records and nursing documentation

Qualifications:

Education: LPN certification/Associates or Bachelors degree in Nursing
Experience: LTC and MDS experience preferred
Skills: Strong attention to detail
Licensure/Certification: LPN LVN RN
Physical Requirements: The ability to sit or stand for extended periods of time and move throughout the facility to monitor resident care
