Why This Role Matters
As a Hospice Intake & Admissions RN you are often the first point of contact on a patients hospice journey. Your work sets the toneproviding comfort clarity and reassurance while guiding patients and families through the transition to hospice care.
What Youll Do
- Serve as a key liaison between patients families providers and our interdisciplinary team
- Review and coordinate referrals to determine hospice eligibility in collaboration with physicians and care teams
- Meet patients where they areat home or in care facilitiesto complete initial assessments
- Initiate care plans including ordering medications medical equipment and supportive services
- Partner with social workers chaplains aides therapists and volunteers to provide holistic care
- Ensure a smooth supportive handoff to the Hospice Case Manager
Where Youll Work
This role offers a dynamic schedule combining:
- Time in our central intake office in Albany
- Field visits to patients in homes nursing homes and assisted living communities across the region
What You Bring
- Active New York State RN license
- ASN required; BSN preferred
- Home care experience (1 year) strongly preferred
- Hospice or end-of-life care experience is a plus
- Valid drivers license and reliable transportation
- Strong communication skillsyou connect easily with patients families and clinical teams
- Clear concise documentation abilities
- Comfort with computers and electronic health records
- Excellent organization and time management skills

PER DIEM RATES based on Monthly Hours Commitments
Pay Range for non-per diem: $38.00-$50.18 Pay is based on experience skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
