Elara Caring is seeking a full-time Registered Nurse to provide home health care in Warwick Rhode Island and surrounding communities.

You will manage your patient visits with independence while receiving support from an experienced clinical team. You will build trusted relationships coordinate individualized care and help patients remain safe and independent in the place they know best: home.

Home health or hospice experience is preferred but nurses with strong transferable clinical experience are encouraged to apply.

• Manage an assigned caseload of home health patients
• Complete initial nursing evaluations and ongoing reassessments
• Develop implement and update individualized plans of care
• Assess patients physical psychological social environmental and caregiver needs
• Provide skilled nursing care in patients homes
• Educate patients and caregivers about medications treatments disease management and safety
• Monitor patient progress and adjust care based on changing needs
• Communicate changes in condition to physicians clinical leadership and other members of the care team
• Coordinate services with nurses therapists physicians caregivers and community resources
• Complete accurate and timely clinical documentation
• Protect patient information and maintain compliance with HIPAA requirements
• Help patients achieve the highest possible level of health safety and independence

• Current and unrestricted Rhode Island RN license
• Bachelors degree in nursing
• At least one year of nursing experience in a clinical care setting
• Strong assessment communication organization and patient-care skills
• Ability to manage an assigned caseload and work independently in the field
• Previous home health or hospice experience is preferred
• Previous Homecare Homebase experience is preferred
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
• Must be able and willing to travel 50%
