As a Registered Nurse Behavioral Health youll contribute to our success in the following ways:

• Assess patients physical emotional social and environmental needs.
• Manage a caseload and develop coordinate and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians supervisors and the care team.
• Update care plans based on patients changing needs.
• Educate patients families and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.

What is Required

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate Diploma Associates or Baccalaureate School of Nursing.
• 1 year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred but not required.
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.

Benefit eligibility can vary by position and shift status.

The base salary for this position is $85000 to $101000 annually based on the companys good faith estimate at the time of posting. Actual pay will be determined based on factors such as education experience skills and internal equity.

This role may also be eligible for additional compensation which could include performance-based bonuses commissions on-call pay or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.
