This is a fast-paced nursing case management role where youll coordinate healthcare services complete clinical assessments and serve as a trusted resource for individuals served and direct support include:Conduct nursing assessments develop and implement healthcare plans and respond to changes in with physicians hospitals pharmacies and other healthcare providers to coordinate physician orders are accurately documented and carried medical oversight training and guidance to direct support in person-centered service planning and advocate for quality healthcare between residential locations using your personal vehicle (mileage reimbursement provided).Maintain timely clinical documentation and meet weekly documentation in a rotating after-hours on-call schedule with one other nurse.
