About the Role
We are seeking a skilled and compassionateHospiceRegistered Nurse Case Manager to manage and coordinate care for an assigned caseload of patients in the home setting. Working under physician orders the Hospice RN Case Manager is responsible for assessing patient needs developing and overseeing individualized plans of care coordinating interdisciplinary services and ensuring continuity of care from admission through discharge.
This role ensures high-quality patient-centered outcomes while maintaining compliance with agency policies accrediting standards and applicable federal and state regulations.
Responsibilities
- Complete admission assessments and ongoing patient evaluations in the home setting.
- Develop implement and update patient plans of care in collaboration with physicians.
- Collect and document OASIS data accurately and on time.
- Provide skilled nursing care while managing multiple patients as assigned.
- Assess manage and educate patients and families on pain medications and disease processes.
- Coordinate care with physicians interdisciplinary team members and agency staff.
- Participate in case conferences and communicate changes in patient condition as needed.
- Maintain timely accurate clinical documentation and comply with agency policies and regulations.
- Uphold patient safety quality standards and confidentiality at all times.
- Complete required trainings meetings and continuing education.
Physical Requirements
- Ability to sit stand and drive for extended periods while providing in-home care.
- Ability to assist and lift patients as well as move equipment or supplies with or without reasonable accommodation.
- Regular use of a tablet for documentation communication and training.

Qualifications
- Active Registered Nurse (RN) license in the state where position is posted.
- Minimum of one (1) year of nursing experience in a hospital facility home health or related healthcare setting (required)
- Home health and hospiceexperience (preferred)
- Valid drivers license current auto insurance and reliable personal transportation (required)
- Ability to work independently in patient homes and manage a flexible schedule (required)
