- Serve as the primary point of contact for prospective clients and families answering inbound inquiries with professionalism compassion and confidence.
- Build trust with families by listening to their needs educating them about our home care services and helping them determine the best next steps.
- Schedule clients for an intake with our Care Managers and ensure each appointment is properly prepared.
- Collect and organize essential client information so Care Managers are equipped for successful intakes.
- Maintain consistent communication with prospective clients throughout the process to provide an exceptional experience.
- Build and nurture relationships with hospitals skilled nursing facilities rehabilitation centers assisted living communities physicians social workers case managers and other referral partners.
- Conduct follow-up calls and outreach to community partners to strengthen relationships and thank them for prospective client opportunities.
- Assist with light marketing initiatives including phone outreach email communication and social media content.
- Prepare intake packets and provide administrative support to the Care Management team.
- Maintain accurate documentation of communication in our CRM.
- Follow up with former clients when services end to better understand their experience and identify opportunities to reconnect.
- Has exceptional communication and interpersonal skills.
- Naturally builds trust and develops meaningful relationships.
- Demonstrates compassion and professionalism when working with families during stressful situations.
- Is highly organized and able to manage multiple priorities in a fast-paced environment.
- Possesses strong problem-solving skills and attention to detail.
- Is comfortable learning and discussing home care services long-term care insurance VA benefits and available resources for seniors.
- Works collaboratively while also taking ownership of responsibilities.
- Is proficient with Google Suite and comfortable learning new .
- Is able to work full-time in the office
- Home care or healthcare
- Community outreach or business development
- Recruiting or relationship management
- Healthcare marketing or physician/community liaison roles
- Social work or care coordination
- Compassionate client-focused demeanor with a commitment to quality care
- Proficiency with care management software or CRM systems a plus
- Knowledge of home care VA Medicare or insurance authorization processes is a bonus
