Job Summary

Make your mark for patients

We are looking for a compassionate bilingual and patient-focused Onward Care Coordinator to join our Rare Disease Patient Support team. This is a remote position aligned with West Coast working hours (PST) supporting Spanish- and English-speaking patients and caregivers throughout their treatment journeys.

About the role

As a Bilingual Onward Care Coordinator you will serve as a primary point of contact for patients enrolled in Rare Disease Patient Support Services. You will deliver individualized high-touch assistance focused on treatment access affordability administration and adherence.

Using your clinical experience and behavioral science skills you will help patients and caregivers navigate barriers identify relevant resources and communicate confidently with their healthcare providers. This is a non-clinical support role and treatment-related or medical questions will be referred to the patients healthcare provider.

Who youll work with

You will collaborate with Rare Case Managers Rare Reimbursement Executives access specialists patient experience colleagues and Rare Disease Patient Support Program operations. Together the team provides consistent compassionate and informed support throughout the patient journey.

What youll do

  • Serve as the primary contact for Spanish- and English-speaking patients and caregivers.
  • Deliver personalized support using empathy active listening and behavioral science skills.
  • Explain insurance coverage access pathways and available affordability support options.
  • Provide non-clinical education and refer medical questions to healthcare providers.
  • Help patients prepare for productive conversations with their healthcare providers.
  • Follow privacy safety adverse-event reporting and program operating requirements.

Interested For this role were looking for following

Minimum requirements

  • Bachelors degree required.
  • Minimum of 6 years of patient-facing clinical experience; or 4 yrs years plus 2 years industry years as a Registered Nurse Doctor of Pharmacy or similarly qualified clinical professional
  • Professional fluency in Spanish and English including speaking reading and writing. Ability to support complex patient and caregiver conversations in both languages
  • Willing and able to work remotely during West Coast business PST hours

Preferred requirements

  • Previous experience working successfully in a remote environment.
  • Patient support program or rare disease experience.
  • Infusion specialty pharmacy or specialty distribution experience.
  • Understanding of insurance coverage and patient financial assistance options.
  • Experience navigating access complexities for newly introduced medications.

This positions reasonably anticipated base salary range is $88000.00 -$115500.00 annually. The actual salary offered will take into account related knowledge skills experience and location among other factors and may fall outside the expected range.
