Salary: $72800- $$83200 annually (depending on experience)
Hourly Equivalent: $35- $40
Shift: Full-Time

Autumn View Health Care Facility is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary this role youll provide clinical leadership coordinate pulmonary-focused care and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

RESPONSIBILITIES:
  • Oversee and coordinate daily operations of the Pulmonary Program
  • Coordinate individualized care plans for residents with pulmonary and respiratory needs
  • Monitor residents with respiratory and pulmonary conditions
  • Collaborate with physicians respiratory providers therapy nursing and interdisciplinary teams
  • Assist with admissions assessments and care coordination for pulmonary residents
  • Provide staff education on pulmonary care and clinical best practices
  • Ensure compliance with regulatory and clinical standards
  • Participate in quality improvement and program development initiatives
REQUIREMENTS:
  • Active New York State Registered Nurse (RN) license required
  • Skilled nursing rehabilitation pulmonary respiratory or subacute experience preferred
  • Strong clinical assessment and care coordination skills
  • Experience with care planning and interdisciplinary collaboration preferred
  • Strong leadership organizational and communication skills
