Salary: $72800- $83200 annually (depending on experience)
Hourly Equivalent: $35- $40
Shift: Full-Time
Garden Gate Health Care Facility is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary this role youll provide clinical leadership coordinate pulmonary-focused care and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

RESPONSIBILITIES:
- Oversee and coordinate daily operations of the Pulmonary Program
- Coordinate individualized care plans for residents with pulmonary and respiratory needs
- Monitor residents with respiratory and pulmonary conditions
- Collaborate with physicians respiratory providers therapy nursing and interdisciplinary teams
- Assist with admissions assessments and care coordination for pulmonary residents
- Provide staff education on pulmonary care and clinical best practices
- Ensure compliance with regulatory and clinical standards
- Participate in quality improvement and program development initiatives
REQUIREMENTS:
- Active New York State Registered Nurse (RN) license required
- Skilled nursing rehabilitation pulmonary respiratory or subacute experience preferred
- Strong clinical assessment and care coordination skills
- Experience with care planning and interdisciplinary collaboration preferred
- Strong leadership organizational and communication skills
