Provider Navigator – Podiatry, Orlando

With more than 25 practitioners our team provides care to patients in skilled nursing facilities assisted living facilities and in the comfort of their own homes across 14 counties.

We are seeking an outstanding full-time Provider Navigator with podiatry experience to join our field team and work closely with our Podiatrist throughout the Orlando and surrounding area.

This is an ideal opportunity for an experienced medical assistant who understands podiatric care geriatric patients and the skilled nursing/assisted living environment and enjoys working in a mobile patient-focused setting.

The Provider Navigator will provide clinical and administrative support to the Podiatrist assist with patient visits coordinate with facility staff and help ensure that patient needs and provider requirements are addressed efficiently.

This position requires travel between skilled nursing facilities assisted living facilities and our clinic throughout the Orlando and surrounding area.

• Travel to skilled nursing and assisted living facilities throughout the Orlando and surrounding area to support the Podiatrist.
• Assist the Podiatrist with patient visits examinations and podiatry-related clinical needs.
• Obtain and document vital signs medical history medications and other relevant patient information.
• Prepare and maintain accurate EMR documentation including H&P information labs medication reconciliation and consent forms.
• Coordinate patient scheduling and daily visits between facilities and the provider.
• Communicate with facility staff regarding physician orders patient needs and follow-up care.
• Maintain patient confidentiality and comply with HIPAA company policies and applicable healthcare regulations.

The ideal candidate will have:

• Previous podiatry experience strongly preferred.
• Experience working with geriatric patients and/or in skilled nursing or assisted living facilities preferred.
• Strong clinical and organizational skills.
• Experience with electronic medical records (EMR).
• Excellent communication and interpersonal skills.
• Ability to work independently while supporting a physician in a mobile environment.
• Strong attention to detail and ability to manage multiple priorities.
• Professional compassionate and patient-centered approach.
• Reliable transportation and the ability to travel between facilities throughout the Orlando and surrounding area.
