PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Family Services team as the Problem-Solving Coordinator at the Family Solution Center Office.
ABOUT PATH
Since its foundation in 1984 PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California connecting our clients to a comprehensive continuum of homelessness prevention street outreach employment preparation and placement assistance individualized case management supportive services interim housing and permanent supportive housing.
ABOUT THE JOB
As part of the Family Services team the Problem-Solving Coordinator also known as Diversion or Rapid Resolution is a crisis response philosophy and approach that focuses on supporting individuals and families experiencing a housing crisis to quickly identify and access alternative housing resources outside of the homeless crisis response system. The Family Problem Solving Coordinator will work to integrate the Problem-Solving Intervention throughout the Family Solution Center.
Position Responsibilities include:
- Work with case managers in interim housing settings to support the integration of Problem-solving in their cases
- Assist with training all FSC staff on Problem-Solving
- Work with the LAHSA Problem-Solving Unit to streamline PSAF requests and intervention updates
- Ensure Problem-Solving Tracking is happening across the FSC Working with key partner family systems to integrate Problem-Solving further upstream from the Coordinated Entry System to reduce in-flow
- Process and accept referrals from funders
- Meet with assigned guests a minimum of 1x/week to support with progression towards goals identified by guest in their Housing Service Plan
- In collaboration with supervisor or MSW/LCSW staff conduct intake and individualized needs assessment for all guests and assist team by collaborating with guests to develop Individualized Service Plans (ISP) that address barriers to obtaining services housing and appropriate health care needs
- Attend and participate in trainings and learning collaboratives as assigned.
- Facilitate psychosocial rehabilitation groups for guests as required
- Conduct a full VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) at admission for all new guests to aid in the Individual Service Plan (ISP) development Conduct and complete all new guest intakes and enter information into HMIS in a timely fashion
- Provide ongoing case management support with benefits establishment (GR SSI TANF Veterans pension etc.) obtaining ID birth certificate social security cards provide employment linkages and assistance housing referrals and health related assistance as identified on the ISP

MINIMUM QUALIFICATIONS
- One (1) year of experience in healthcare case management homeless services or working with vulnerable populations or a related field.
MANDATORY REQUIREMENTS
Regardless of qualifications candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Successfully complete the following as a condition of hire:

Driving is an essential function of this position. To meet the employment requirements for this role all candidates must:
- Have reliable transportation
