Are you an experienced Home Health RN looking for a part-time position that offers flexibility autonomy and meaningful one-on-one patient care

Elara Caring is seeking a compassionate PRN Registered Nurse to provide skilled nursing care to patients in their homes. This role is ideal for an RN who understands home health operations can work independently and values building meaningful relationships with patients and families. PRN scheduling is available but dependable and consistent availability is required (whether that is 4 hours/week or 12 hours/week).

• Complete admissions assessments routine visits recertifications and discharges

• Develop and update individualized plans of care

• Monitor patients conditions and communicate significant changes

• Coordinate care with physicians caregivers and the interdisciplinary team

• Educate patients and families about medications conditions safety and self-care

• Complete accurate and timely clinical documentation

• Help prevent avoidable hospitalizations through proactive nursing care

• Current unrestricted Massachusetts RN license

• Previous home health nursing experience required

• Ability to work independently and manage visits efficiently

• Availability to commit to a consistent part-time schedule

• Valid drivers license reliable transportation and current auto insurance

• Willingness to travel throughout Bellingham and surrounding communities

• Ability to sit stand bend lift and move intermittently and lift 50100 pounds
