Elara Caring is seeking a compassionate Physical Therapist to serve patients throughout the New Haven this field-based role you will provide personalized one-on-one therapy while helping patients improve their safety independence and quality of life.

You will have the autonomy to manage your patient visits while receiving support from an experienced and collaborative clinical team.

• Evaluate patients mobility strength safety and functional needs
• Develop and implement individualized treatment plans
• Provide skilled physical therapy services in patients homes
• Help patients improve mobility and safely perform daily activities
• Educate patients and caregivers on exercises equipment and safety techniques
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians caregivers nurses therapy professionals and other members of the care team
• Communicate changes in patient condition to the appropriate clinical team members
• Complete accurate and timely clinical documentation
• Supervise therapy assistants and support staff as appropriate
• Support patients in reaching their highest possible level of independence

• Graduate of an American Physical Therapy Association-approved physical therapy program
• Current and unrestricted Connecticut Physical Therapist license
• Minimum of one year of experience in a healthcare facility
• Strong communication organization and patient-care skills
• Ability to work independently in a home-based environment
• Willingness to travel throughout the assigned Norwalk and Fairfield territory
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment

The base salary for this position is$100000 to $115000 annually based on the companys good faith estimate at the time of posting. Actual pay will be determined based on factors such as education experience skills and internal equity.
