As a Physical Therapist you will:
- Evaluate patients and develop individualized treatment plans
- Provide skilled physical therapy services
- Help patients improve strength balance mobility and functional independence
- Monitor patient progress and modify treatment plans as needed
- Collaborate with nursing physicians families and the interdisciplinary team
- Supervise and collaborate with Physical Therapist Assistants
- Participate in care planning and discharge planning
- Educate patients families and facility staff on appropriate therapeutic programs
- Complete accurate and timely clinical documentation
- Graduate of an accredited Physical Therapy program
- Current Michigan Physical Therapist license
- Strong clinical and communication skills
- Compassionate patient-centered approach
- Ability to work independently and as part of a team
- Skilled nursing/post-acute experience is preferred but not required
