Job Summary
Vantage Health Partners is seeking a dedicated Physical Therapist to join our Hospital/Health Care team in Hamilton. This part-time on-site position offers a rewarding opportunity to deliver patient-centered rehabilitation services within a collaborative clinical environment. In this role you will work directly with patients to assess their physical conditions develop individualized treatment plans and guide them through therapeutic exercises and interventions. - Evaluate patients physical conditions and functional limitations
- Develop and implement individualized physical therapy treatment plans
- Administer therapeutic exercises manual therapy and modalities
- Monitor patient progress and adjust treatment approaches accordingly
- Educate patients and families on home exercise programs and injury prevention
- Collaborate with physicians nurses and other health care professionals
- Maintain accurate and timely patient documentation and records
- Ensure compliance with hospital policies and regulatory standards
- Assist patients in restoring mobility and managing chronic pain
- Participate in care coordination meetings and clinical discussions
- Stay current with advancements in physical therapy practices
- Associate degree in Physical Therapy or related field
- Valid state license or certification to practice as a Physical Therapist
- Experience in a hospital or clinical health care setting
- Strong knowledge of anatomy kinesiology and rehabilitation techniques
- Proficiency in therapeutic exercises and manual therapy methods
- Ability to assess patient conditions and develop effective treatment plans
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Compassion and empathy for patients undergoing rehabilitation
