Physical Therapist Outpatient

We are hiring a dedicated and compassionate full-time Physical Therapist to join our growing outpatient rehabilitation team delivering high-quality resident-centered care in a collaborative environment.

All of our residents live within our award-winning campus creating an environment where you can be part of their journey throughout their continuum of care. Our Physical Therapists provide a holistic approach supporting the residents physical spiritual emotional and clinical needs. They are responsible for implementing residents personal care plans focusing on residents strengths preferences and preferred routines while providing a sense of comfort companionship and belonging.

Our communities offer levels of care and include specialty programs such as: urinary incontinence vestibular rehab home safety assessment movement disorders and low vision.

All of our clients live within our community which means no additional travel!

What we offer:

• Compensation: $42 - $50 per hour commensurate with experience

How you will make an impact:

• Perform physical therapy screenings evaluations and treatments across multiple levels of care.

• Develop and implement individualized treatment plans to improve residents function and quality of life.

• Collaborate with physicians residents families caregivers and interdisciplinary team members.

• Maintain accurate clinical documentation and billing in compliance with regulatory and reimbursement requirements.

• Educate residents caregivers and staff on therapy goals and interventions.

• Support a safe respectful and resident-focused environment.

What you will need:

• Must possess a valid and current Maryland State Physical Therapy license

• Must meet current Medicare and state requirements

• Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS etc.)

• Previous Physical Therapy experience

• Therapeutic experience in older adults across multiple diagnostic groups is preferred
