Physical Therapist (PT) Home Health Olean NY PRN
Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide one-on-one care to patients throughout Olean NY and surrounding communities.
This opportunity is ideal for a Physical Therapist looking to supplement an existing schedule maintain a small caseload or add flexible home health work while maintaining control over when and where they see patients.
With a minimum commitment of just 5 patients our PRN option provides flexibility while allowing therapists to build a geographically concentrated caseload within the Olean area.
What Youll Do:
As a Physical Therapist with Stern at Home Therapy youll evaluate and treat a primarily geriatric patient population in their homes.
Youll maintain clinical autonomy over evaluations and treatment plans while our administrative team assists with referrals scheduling authorizations and patient coordination.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients homes.
- Help patients improve strength balance mobility endurance and functional independence.
- Provide gait training transfer training therapeutic exercise and fall-prevention strategies.
- Recommend assistive devices durable medical equipment and home safety modifications when appropriate.
- Educate patients and caregivers on exercises mobility strategies and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians caregivers families and interdisciplinary care team members.
Compensation:
- PRN 1099 Minimum 5 Patients.
- $62 per follow-up visit.
- $75 per evaluation.
- Fixed per-visit compensation with no tiered volume model.
- PRN positions are 1099 independent contractor roles only.

Qualifications:
- Active Physical Therapist license in New York required.
- Graduate of an accredited Physical Therapy program.
- Reliable transportation required.
- Strong clinical communication documentation and time-management skills.
- Ability to work independently in a home-based environment.
- Home health or geriatric experience preferred but not required.
