Physical Therapist

There is currently an urgent need for a licensed PT to join a premier highly-respected health care organization on a full-time basis. This is a terrific opportunity offering a lucrative base salary great benefits package and other outstanding perks. This role would travel to assisted living facilities and patient homes to provide treatment. The territory is a 50 mile radius around the Fond du Lac Area and no overnight travel would be need.

Job Description

• Participate in the development of patient physical therapy plans of care
• Make visits to patients for assessment/evaluations and communicate with physicians before start of care and as needed
• Implement the plans of care and evaluate outcomes
• Document patient care
• Provide therapeutic treatments and evaluation of equipment need as applicable.
• Provide skilled restorative treatment for pain reduction and develop/restore/maintain functions

All candidates MUST have the following experiences to qualify for consideration
