Job Summary
Physical Therapist (PT) Home Health Buffalo NY Full-Time & Part-Time 3 Openings.
Stern at Home Therapy is expanding throughout the Buffalo NY area and is hiring three Physical Therapists (PTs) to support our continued growth.
We offer opportunities across a broad territory from North Buffalo through the city and down into the Southtowns allowing clinicians to build a caseload closer to home rather than covering the entire region. The Southtowns generally include communities such as West Seneca Orchard Park Hamburg East Aurora and surrounding areas.
We are hiring for Full-Time and Part-Time making this a strong opportunity whether youre building a primary caseload or adding flexible visits around an existing position.
What Youll Do:
As a Physical Therapist with Stern at Home Therapy youll evaluate and treat a primarily geriatric patient population in their homes.
Youll have the clinical autonomy to develop individualized plans of care while our administrative team supports you with referrals scheduling authorizations and patient coordination.
With three openings available were building coverage across different sections of the Buffalo market so clinicians can develop geographically concentrated caseloads.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients homes.
- Help patients improve strength balance mobility endurance and functional independence.
- Provide gait training transfer training fall-prevention strategies and home exercise programs.
- Recommend assistive devices durable medical equipment and home safety modifications when appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians caregivers families and the interdisciplinary care team.
- Coordinate with our administrative team to develop a caseload within your preferred Buffalo-area territory.
Compensation: Full-Time Minimum 16 Patients.
W-2 Tiered Per-Visit Option:
- $45$60 per follow-up visit.
- $65 per 4-unit evaluation.
- Your per-visit rate increases as your monthly patient volume grows.
1099 Tiered Per-Visit Option:
- $60$70 per follow-up visit.
- $75 per evaluation.
- Your per-visit rate increases as your monthly patient volume grows.
Part-Time Minimum 10 Patients.
W-2:
- $55 per follow-up visit.
- $65 per 4-unit evaluation.
1099:
- $62 per follow-up visit.
- $75 per evaluation.
Full-Time W-2 Benefits:
- 401(k) with annual match employer match starts at 1.5% and increases up to 7%.

Qualifications:
- Active Physical Therapist license in New York required.
- Graduate of an accredited Physical Therapy program.
- Reliable transportation required.
- Strong clinical communication documentation and time-management skills.
- Ability to work independently in a home-based setting.
- Home health or geriatric experience is preferred but not required.
