Job Summary

As a Pharmacy Technician Care Manager you will work closely with healthcare teams and patients to optimize outcomes and improve overall health.


Responsibilities (RE)
  • Conduct medication reconciliation and resolve discrepancies with care teams
  • Educate patients on medication use side effects and adherence strategies
  • Monitor medication adherence and follow up with at-risk patients
  • Support care transitions including refills prior authorizations and delivery coordination
  • Maintain accurate documentation in EHR systems
  • Collaborate with pharmacists to identify drug interactions and improve care plans
  • Ensure compliance with HIPAA and pharmacy regulations
  • Participate in quality improvement initiatives


Requirements

Qualifications

  • Certification: Current Pharmacy Technician License from a recognized certifying body (e.g. PTCB or NHA).
  • Experience: Previous experience in a pharmacy setting ideally with a focus on care management or chronic disease management.
  • Knowledge: Strong understanding of pharmacology medication therapy management (MTM) and patient-centered care practices.

  • Technology Proficiency: Familiarity with EHR systems pharmacy software and telehealth tools.
  • Communication: Excellent verbal and written communication skills for patient and provider interactions.
  • Attention to Detail: Exceptional accuracy in documenting patient information and managing medication orders.
