Pharmacist
Duties:
- Review and verify medication orders for accuracy appropriateness and potential drug interactions.
- Provide clinical pharmacy support to physicians nurses and other healthcare professionals.
- Monitor medication therapy and recommend adjustments when appropriate.
- Counsel patients and healthcare staff regarding medications dosage administration and potential side effects.
- Participate in medication reconciliation and patient-care planning.
- Maintain accurate pharmacy records and documentation..
Required Qualifications:
- Licensed in the State of Iowa as a Registered Pharmacist required
- Graduate of an accredited college of pharmacy required
- 24 years of clinical pharmacy experience in a hospital setting required
- Strong knowledge of medication management pharmacotherapy and patient safety.
- Strong written and verbal communication skills.
