Job Description

We are seeking a dedicated self-motivated and compassionate Registered Nurse (RN) to join our home care team in the Lawrence this role you will perform essential clinical visits for Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) clients focusing on initial assessments admissions Start of Care (SOC) and ongoing supervisory visits to ensure the highest standards of care.

This position offers excellent flexibility and autonomy making it ideal for an RN seeking supplemental income with a manageable weekly workload.

Key Responsibilities

  • Admissions & Assessments: Conduct comprehensive nursing assessments and admissions for AFC and GAFC clients in accordance with MassHealth regulations and agency guidelines.

  • Start of Care (SOC): Initiate Start of Care visits evaluating client physical and psychosocial needs to formulate individualized client-centered care plans.

  • Supervisory Visits: Perform regular supervisory visits to evaluate client health status monitor caregiver effectiveness and update care plans as needed.

  • Documentation: Maintain accurate timely electronic clinical documentation nursing assessments and progress notes following each visit.

  • Collaboration: Communicate and collaborate effectively with clinical management care coordinators caregivers physicians and multidisciplinary team members.

Qualifications & Requirements

  • Education & Credentials:

  • Active unencumbered Registered Nurse (RN) license in the state of Massachusetts.

  • Current CPR / BLS certification.

  • Experience:

  • Minimum 12 years of nursing experience (Home Health Home Care or AFC/GAFC experience strongly preferred).

  • Knowledge of MassHealth AFC/GAFC regulations and clinical assessment standards.

  • Skills & Other Requirements:

  • Valid drivers license reliable personal vehicle and active auto insurance.

  • Strong physical assessment clinical judgment and critical-thinking skills.

  • Ability to work independently and manage time effectively.

  • Excellent written and verbal communication skills.
