Currently we are looking for a full time Patient Care Coordinator for our Banner Court location in Modesto.

Schedule: Monday - Thursday 7:00am - 5:00pm & Friday 8:00am - Noon Friday


POSITION SUMMARY
The Patient Care Coordinator Float (Offsite) is responsible for providing centralized administrative and front-office support to assigned TPG clinics from the Modesto HQ location. This position performs essential non-patient-facing PCC workflows including patient scheduling telephone support assigned clinic tasks and other administrative functions necessary to support clinic operations and patient access.


The PCC Float will partner closely with clinic and regional leadership to provide coverage based on operational needs assist with high-volume workflows and ensure assigned responsibilities are completed accurately and timely in accordance with TPG policies procedures and HIPAA requirements.

Strong customer service communication organization and computer skills are essential for this role. The individual must be comfortable supporting multiple clinics and priorities and communicatingprofessionally with patients clinical staff corporate employees and external contacts.

KEY RESPONSIBILITIES
Minimum Position Requirements
  • Communicates with patients staff and external contacts in a friendly courteous and professional manner.
  • Provides centralized administrative support to assigned TPG clinics based on operational and staffing needs.
  • Answers transferred clinic phone calls with professionalism and care when the clinic PCC is unavailable or additional phone coverage is needed.
  • Returns assigned voicemails timely with the expectation that voicemails received during the workday are addressed the same day.
  • Maintains a phone answer rate of 85% or higher when assigned to phone coverage.
  • Schedules patient initial evaluations and follow-up visits through the established plan of care.
  • Assists with new-patient scheduling and collection of necessary patient information.
  • Accurately enters and maintains patient information and demographics within applicable systems.
  • Calls and confirms patient appointments as assigned.
  • Follows up on no-show patients and assists with rescheduling appointments as assigned.
  • Assists with treatment authorization and/or continued treatment prescription follow-up when assigned.
  • Completes high-volume assigned clinic tasks scheduling work queues and other essential non-patient-facing PCC workflows.
  • Supports multiple clinics and adjusts priorities based on daily operational needs and direction from leadership.
  • Works closely with clinic leadership Regional Front Office leadership Billing and other departments to support efficient patient and administrative workflows.
  • Escalates patient issues concerns or situations requiring clinic-level intervention to the appropriate Clinical Director or Regional Front Office leadership.
  • Maintains patient confidentiality and protects confidential information at all times.
  • Participates in and adheres to TPG HIPAA policies and procedures.
  • Performs other administrative duties and PCC support functions as assigned.
QUALIFICATIONS
Skills and Competencies
  1. Thorough knowledge of Microsoft Office; Raintree or applicable Practice Management System experience preferred.
  2. Strong customer service and relationship-building skills.
  3. Strong organizational skills with the ability to manage multiple priorities and support multiple clinic locations.
  4. Ability to work independently in an offsite environment while maintaining effective communication with clinic and regional leadership.
  5. Ability to maintain confidential patient and business information.
  6. Ability to communicate information clearly and professionally both verbally and in writing.
  7. Ability to demonstrate active listening ask clarifying questions identify and share relevant information and receive feedback objectively.
  8. Strong problem-solving skills and ability to appropriately escalate issues.
  9. Ability to adapt to changing clinic needs workloads and priorities.
  10. Strong team-player mentality and willingness to provide coverage where operationally needed.
Education
High school diploma or equivalent.
Must be 18 years of age or older.

Experience
Minimum of 1 year of consistent experience as a medical receptionist or in a comparable front office role preferred. Experience with patient scheduling practice management systems and insurance plans strongly preferred.

Physical Requirements
Requires prolonged sitting and regular use of a computer and telephone. Requires some walking bending stooping reaching and stretching. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard mouse telephone photocopier calculator and other standard office equipment. Requires normal range of hearing and eyesight to communicate record information prepare documentation and perform assigned responsibilities.
