Part Time Case Manager

The Eagle Mountain Family Clinic provides a variety of therapeutic services for clients affected by trauma and/or mental illness. As a Case Manager youll help clients access community resources coordinate services and connect with the support they need while working alongside a collaborative multidisciplinary team to help clients build stability confidence and progress toward their personal goals.

What Youll Do as a Case Manager

• Conduct client assessments and help create individualized care plans.
• Coordinate services and collaborate with other providers to support client needs.
• Offer ongoing support guidance and follow-through with clients.
• Connect clients and families with community resources and supportive services.
• Work closely with a supportive multidisciplinary team.
• Help clients navigate appointments services and other resources.
• Build positive relationships with clients and families through compassionate client-centered support.
• Help clients build stability confidence and progress toward personal goals.
• Maintain accurate documentation case notes and client records.

Qualifications

Applicants must meet ONE of the following:

Note: Applicants under the working toward SSW pathway must obtain their SSW license within 24 months of hire.

• Bachelors degree with Social Service Worker (SSW) license
• Bachelors degree in any field with willingness to pursue SSW licensure

Additional Requirements:

• Valid drivers license
• Basic understanding of Social Work principles
• Familiarity with social welfare policy and human development
• Ability to problem-solve and maintain a client-centered approach
• Spanish speakers may qualify for increased wages after testing

Employment at Wasatch Behavioral Health is contingent upon successful completion of a drug screen motor vehicle record review and criminal background check. After hire you must complete and receive a Criminal Background Clearance (BCI) from Utah State Office of Licensing. Upon receipt of a conditional offer of employment in the position you must provide an official copy of licensure or certification.
