What You Will Do:
- Build respectful person-centered relationships that promote recovery self-determination choice dignity and community inclusion.
- Support individuals with activities of daily living and help them develop and maintain skills needed to live as independently as possible.
- Implement individualized service plan goals and provide timely updates on progress barriers and changing needs to the Outreach Coordinator Supervisor and multidisciplinary team.
- Assist with scheduling transportation and follow-up for medical behavioral health benefits housing employment education and community appointmentsincluding providing transportation in your own vehicle as needed.
- Monitor the health mental status and safety of individuals served promptly reporting significant changes or risks.
- Assist with medication support and documentation within your training and authorization in accordance with BHN policy and MAP requirements.
- Provide crisis intervention and emergency support as trained following required notification and escalation procedures.
- Help individuals build and maintain natural supports family connections and community and cultural engagement.
- Support individuals with practical needs such as benefits identification documents housing subsidies budgeting and referrals.
- Provide hands-on assistance with household tasks meal planning and daily living activities as part of individualized service goals.
- Complete accurate timely documentation in the electronic health record including progress notes incident reports and Medication Occurrence Reports (MORs).
- Use persistence and creative outreach approaches to maintain engagement with individuals who are difficult to reach.
- Perform other duties as assigned.
Who You Are:
- High school diploma or GED required.
- Minimum of six months of human services experience preferred; BHN provides position-specific training.
- Committed to individual empowerment recovery human rights and community integration.
- Strong organizational interpersonal and communication skills with the ability to manage multiple tasks and respond to urgent needs.
- Comfortable working in a variety of community-based settings including private homes vehicles and outdoor environments.
- Valid drivers license and a reliable registered and insured vehicle available for all working hours required.
