Occupational Therapist (OT) Mobile Outpatient Niagara County NY Part-Time
Stern at Home Therapy is hiring a Part-Time Occupational Therapist (OT) to provide individualized one-on-one care to patients throughout Niagara County NY.
This is an excellent opportunity for an Occupational Therapist looking for a consistent part-time caseload flexible scheduling and strong clinical autonomy while treating patients one-on-one in their homes.
With a minimum commitment of 10 patients therapists can build a geographically concentrated caseload within Niagara County while receiving dedicated administrative support for referrals scheduling authorizations and patient coordination.
What Youll Do:
As an Occupational Therapist with Stern at Home Therapy youll evaluate and treat a primarily geriatric patient population in their homes helping patients improve or maintain independence with daily activities and functional tasks.
Responsibilities Include:
- Perform occupational therapy evaluations and develop individualized plans of care.
- Provide one-on-one occupational therapy treatments in patients homes.
- Help patients improve independence with activities of daily living.
- Address upper-extremity strength range of motion coordination cognition safety and overall functional performance.
- Provide therapeutic activities and exercises based on individual patient goals.
- Recommend adaptive equipment assistive devices and home modifications when appropriate.
- Educate patients and caregivers on adaptive strategies exercises and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians caregivers families and interdisciplinary care team members.
Compensation:
Part-Time Minimum 10 Patients
Part-Time W-2:
- $55 per follow-up visit.
- $65 per 4-unit evaluation.
- Fixed per-visit compensation with no tiered volume model.
Part-Time 1099:
- $62 per follow-up visit.
- $75 per evaluation.
- Fixed per-visit compensation with no tiered volume model.
Part-Time W-2 Benefits:
- Health insurance and/or medical stipend based on eligibility.

- Prorated commuter stipend based on patient volume up to $175 per month for clinicians seeing 10 patients.

Qualifications:
- Active Occupational Therapist license in New York required.
- Graduate of an accredited Occupational Therapy program.
- Reliable transportation required.
- Strong clinical communication documentation and time-management skills.
- Ability to work independently in a home-based environment.
- Home health or geriatric experience preferred but not required.
