Your Responsibilities Include:

• Delivering personalized high-quality care to seniors in their homes assisted living hospice and nursing facilities.
• Delivering dementia-informed relationship-based care across the continuum of care.
• Conducting comprehensive assessments and developing tailored care plans for older adults.
• Managing chronic conditions and performing care with skill and empathy for patients and their families.
• Collaborating with cross-functional partners including palliative psych and neuro to deliver coordinated whole-person care.
• Building trusted relationships with residents caregivers and facility partners to enhance engagement and referrals.
• Maintaining consistency in documentation follow-through and patient communication.

What Youll Bring

• Active unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate patient-centered approach and strong clinical judgment.
• Valid drivers license and reliable transportation.
