Key Responsibilities:

• Deliver comprehensive primary care services including preventive screenings diagnosis and treatment.
• Integrate SDOH screenings and interventions into patient assessments and care plans.
• Participate in continuous quality improvement initiatives to meet and exceed PCMH quality metrics.

What Youll Bring

• Active NP license and DEA certification for Louisiana.
• Experience in primary care acute care or urgent care preferred.
• Strong critical thinking communication and independent decision-making skills.
• Passion for flexible patient-centered care in metro and under-served areas.
