This is a part time benefit eligible position working 3 days per week for a total of 24 hours within our internal medicine outpatient clinic - adults (18) with a focus on prevention and disease management. Scheduled 3 days flexible potential of one late night.

OVERVIEW OF POSITION: Under the direction of the Chief Medical Officer (CMO) or applicable Medical Director and with guidance from a collaborating physician the Nurse Practitioner or Physician Assistant is responsible for medical care consistent with accepted professional standards.


Must adhere to the organizational Policies Procedures Code of Ethics and all licensing and accreditation standards of the position. Must represent the organizations Mission in all actions and communications.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide excellent direct patient care in a comprehensive compassionate and team-based manner in an internal medicine outpatient setting.
  • Assist in the coordination and/or supervision of prevention services for patients.
  • Provide medical evaluation and treatment of ill patients based on scientific knowledge.
  • Provide medical care to all patients within the expectations of the specialty hired into:
    • Internal Medicine: Primary care for adults (18) with a focus on prevention and disease management.
  • Maintain accurate and detailed medical records about each patient.
  • Prescribe medication referral and diagnostic studies when indicated.
  • Recommend lifestyle changes as appropriate to improve health and quality of life.
  • Follow and interpret diagnostic results and ensure that the patient is notified of these results and any further steps that may be needed.
  • Promote team-based care to better serve patients. This includes and is not limited to: MA Nursing staff RD Pharmacist Community Health Workers Behavioral Health Optometrists Dentists.
  • Provide coverage for patients when other providers take leave.
  • Participate in rotation of after-hours call.
    • (approximately 5 weeks per year dependent on departmental staffing; calls are initially triaged by Corewell Health nurses before being routed to the provider).
  • Participate in regular chart review with collaborative physician per collaborative physician agreement.
  • Participate in the peer review process.
  • Follow clinic policies procedures and protocols to achieve Cherry Street Services Inc. goals and objectives.
  • Strive to achieve high quality metrics under the direction of Cherry Healths Quality Assurance program.
  • Serve as a liaison with the medical community and establish and maintain linkages with external agencies and organizations.
  • Attend regular internal and external departmental meetings to ensure progress in meeting clinical program and agency goals.
  • Understand and adhere to CSHSs compliance standards as they appear in CSHSs Corporate Compliance Policy Code of Conduct and Conflict of Interest Policy including HIPAA.
  • Keep abreast of all pertinent federal state and CSHSs regulations laws and policies as they presently exist and as they change or are modified.
  • Other duties as assigned.

SKILLS / KNOWLEDGE / ABILITIES:

  • Basic Life Support/CPR training according to requirements of license and/or certification.

EDUCATION / LICENSES / CERTIFICATES: Current nurse specialty certification by the Michigan State Board of Nursing current certification by professional nursing organization e.g. ANA NAACOC and current license to practice nursing in Michigan.


EXPERIENCE: Two years experience working in an internal medicine or primary care clinic setting or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above.


PHYSICAL REQUIREMENTS: Possible exposure to patients/clients with illnesses and/or infections. Frequent telephone and computer use requiring manual dexterity for writing and computer use. Sitting standing or walking for extended periods of time. Specific vision abilities required by this job may include close vision distance vision and ability to adjust focus. May require bending squatting or kneeling and lifting up to 50 pounds. Use of Personal Protective Equipment (PPE) as required.


CLIENT POPULATION SERVED: Adult outpatient patient population (ages 18 and older) consistent with internal medicine specialty.
