As a Nurse Practitioner at Vantage Health Partners you will play a vital role in providing comprehensive primary and specialty care to patients across our community. Working alongside physicians nurses and allied health professionals you will assess diagnose and treat patients while developing personalized care plans that promote wellness and improve health outcomes. We offer a competitive salary range of $100000 - $120000 yearly along with a supportive work culture and opportunities to grow professionally within our dynamic healthcare setting. - Conduct comprehensive patient assessments including physical exams and medical histories
- Diagnose and treat acute and chronic health conditions under physician collaboration
- Order and interpret diagnostic tests such as lab work imaging and screenings
- Develop and implement individualized patient care plans
- Prescribe medications and treatments in accordance with state regulations
- Educate patients and families on health management prevention and wellness strategies
- Collaborate with physicians nurses and multidisciplinary teams to coordinate care
- Maintain accurate and timely electronic health records documentation
- Monitor patient progress and adjust treatment plans as needed
- Participate in quality improvement initiatives and clinical protocols
- Provide guidance and support to nursing staff and clinical trainees
- Ensure compliance with healthcare regulations and safety standards
- Associate degree in Nursing or related healthcare field
- Valid Nurse Practitioner licensure and board certification
- Clinical experience in a hospital or healthcare setting
- Strong knowledge of patient assessment diagnosis and treatment protocols
- Proficiency in electronic health record systems
- Excellent communication and interpersonal skills
- Ability to work collaboratively within a multidisciplinary team
- Strong critical thinking and clinical decision-making abilities
- Compassion and commitment to patient-centered care
- Ability to manage multiple patients and priorities in a fast-paced environment
- Preferred: Experience in primary care or specialty clinical practice
