Cornerstone Caregiving of Wooster is growing our 24/7 Assessment Care Team and is seeking experienced dependable caregivers who are passionate about providing exceptional care and support when clients need it most.

What Is a 24/7 Assessment

Our Assessment Caregivers provide around-the-clock support and observation during critical periods in a clients care journey. These assignments often occur following a hospital discharge a change in condition or when additional support is needed to evaluate safety abilities and overall care needs.

This role is ideal for caregivers who can work independently communicate effectively and provide detailed observations that help our team deliver the highest quality care.

We Are Looking for Caregivers Experienced With:
Personal care and total care assistance
Memory care and dementia support
Safe transfers and mobility assistance
Fall prevention and safety monitoring
Toileting bathing and activities of daily living (ADLs)
Meal preparation and light housekeeping
Monitoring and reporting changes in client condition
Accurate and detailed documentation
Providing thorough shift reports to the management team
Working independently and exercising sound judgment
Requirements
Valid drivers license
Reliable vehicle and current auto insurance
Ability to pass required background screening
Dependable transportation
Strong communication and reporting skills
Excellent attention to detail
Reliable attendance and professionalism
What We Offer
Competitive pay from $16.00 to $22.50 per hour based on shift and applicable differentials
