Job Summary:
Tryfacta is seeking a LVN / LPN LTC for our client in Vestal NY 13850. This is a 16 weeks contract assignment. If you meet the qualifications listed below and are interested please Apply Now!
Job Details:
- Position: LVN / LPN LTC
- Location: 3700 Vestal Rd Vestal NY 13850
- Contract Duration: 16 weeks
- Start Date: Start dates available every 2 weeks based on orientation schedule
- Shift: Day / Evening / Night
- Schedule: 5x8 or 3x12
- Pay Rate: $40$45/hr
- Patient Ratio: Approximately 1:1820
- Facility Size: 300 beds
- Weekend Requirement: Every other weekend as requested by the center
- Charting: PointClickCare (PCC)
- Parking: Free
- Mileage Reimbursement: None
- Scrubs: Any solid color; closed-toe shoes required
- Locals: Accepted

Responsibilities
- Administer medications and treatments as prescribed.
- Perform medication passes accurately and safely.
- Monitor residents and report changes in condition.
- Complete timely and accurate nursing documentation.
- Provide nursing care according to facility policies and procedures.
- Supervise CNAs on assigned units.
- Maintain strong attendance and punctuality.
Requirements
- Active New York State LVN/LPN license required.
- No license hits allowed.
- Proof of active license must be provided at the time of submission.
- Minimum 1 year of LTC / Skilled Nursing experience required.
- BLS certification required.
- Must be able to successfully complete a medication administration (med pass) test and clinical observation.
- First-time travelers are accepted.
- Must be dependable and maintain consistent attendance.
- Must be able to work in a fast-paced environment.
- Must be flexible with scheduling needs.
Important Assignment Requirements
- Shift cannot be changed once offered.
- RTO must be submitted and approved at the time of submission.
- RTO cannot be added after an offer is made.
- Cannot request every holiday off.
- Any call-outs must be communicated with ample notice preferably more than 24 hours in advance.
- Compliance process typically takes 1.52 weeks.
- No floating between like units is expected.
Modules & Compliance
Once the offer is accepted the client will send Relias module access via email within 48 hours. Candidates should check their spam/junk folder if the email is not received.
Modules must be started as soon as possible and completed no later than 3 days before the assignment start date.
