Schedule: Day Shift 36 Hours Guaranteed 13 Weeks
Shift Timing: 5:00 AM 5:30 PM (30-minute meal break)
Working Days: Monday Tuesday Wednesday

A skilled nursing and rehabilitation facility in Bloomfield New Mexico is seeking an experienced LPN/RN for a 13-week local contract assignment. The role involves providing direct patient care in a long-term care and rehab setting ensuring safe high-quality and patient-centered nursing care.

• Provide direct nursing care to residents under clinical supervision
• Administer medications and treatments as prescribed
• Monitor assess and document patient conditions and vital signs
• Support care plans and rehabilitation goals
• Assist with activities of daily living (ADLs) as needed
• Communicate patient condition changes to healthcare team
• Ensure patient safety comfort and dignity at all times
• Maintain accurate clinical documentation
• Follow infection control and facility safety protocols
• Collaborate with interdisciplinary care teams

• Active LPN or RN license required
• Prior experience in long-term care or rehabilitation preferred
• Strong clinical assessment and medication administration skills required
• Ability to monitor and document patient conditions accurately required
• Knowledge of infection control and safety procedures required
• Strong communication and documentation skills required
• Ability to work in a structured healthcare environment required
