• Provide skilled nursing care to patients and residents in a facility setting

• Administer medications and treatments as ordered and within your scope of practice

• Monitor patients for changes in condition and communicate concerns to the appropriate members of the care team

• Complete accurate and timely clinical documentation

• Assist with assessments and contribute to individualized plans of care

• Communicate effectively with RNs LNAs providers patients residents and families

• Promote patient safety comfort dignity and quality of care

• Follow facility policies procedures and applicable nursing standards

• Active New Hampshire LPN license

• Previous facility long-term care rehabilitation or similar healthcare experience preferred

• Strong clinical judgment and attention to detail

• Dependable professional and compassionate approach to patient care

• Excellent communication and teamwork skills

• Ability to adapt quickly to different facility environments

• Comfortable working independently while collaborating with the broader care team
