Licensed Practical Nurse (LPN) Clinical Coordinator

Schedule: MondayFriday 8:00 AM 5:00 PM In-Office Position

What Youll Bring
  • Current LPN/LVN license

  • Minimum of 2 years of LPN/LVNexperience preferred

  • Home health experience preferred

  • Knowledge of Medicare and Medicaid regulations benefits and home health practices preferred

  • Strong organizational and time-management skills

  • Excellent communication and interpersonal abilities

  • Positive patient-centered approach to care

  • Commitment to delivering exceptional clinical outcomes and service
