Schedule: MondayFriday 8:00 AM 5:00 PM In-Office Position

We are seeking a dedicated Licensed Practical Nurse (LPN) Clinical Coordinator to join our growing this role youll play a vital part in coordinating patient care supporting clinical operations and ensuring patients receive the right care at the right timeall while making a meaningful impact in the lives of those we serve.

• Current LPN/LVN license

• Minimum of 2 years of LPN/LVNexperience preferred

• Home health experience preferred

• Knowledge of Medicare and Medicaid regulations benefits and home health practices preferred
