• Saturday & Sunday 7:00 AM-7:00 PM
• Saturday & Sunday 7:00 PM-7:00 AM

Pay: Starting at $27.00/hour based on experience

Sign-On / Retention Bonus: $5000 sign-on and retention bonus; paid at 90 days 6 months and 1 year of employment

Work Setting: On-site at an Adult Crisis Therapeutic Home (ACTH) serving adults with intellectual and developmental disabilities (I/DD) and co-occurring behavioral health needs

Do you want work that makes a meaningful difference in your community Easterseals PORT Health is seeking a Licensed Practical Nurse (LPN) to join our Virginia REACH Adult Crisis Therapeutic Home (ACTH) team in Woodbridge Virginia.

The Virginia REACH Adult Crisis Therapeutic Home provides short-term crisis stabilization services for adults with intellectual and developmental disabilities (I/DD) experiencing behavioral health a supportive home-like setting individuals receive person-centered care clinical support and skill-building services designed to promote stability prevent hospitalization and support successful transitions back to community living. Through therapeutic interventions and interdisciplinary collaboration the program helps individuals achieve greater independence wellness and quality of life.

As a Licensed Practical Nurse youll provide direct nursing care and medical support to individuals receiving crisis stabilization services. Youll administer medications conduct nursing assessments provide health education and collaborate with interdisciplinary team members to ensure high-quality care that supports both physical and behavioral health needs.

What Youll Do

• Provide nursing services and medical support to individuals with I/DD and co-occurring behavioral health needs in a crisis stabilization setting.
• Administer medications safely and accurately in accordance with physician orders and clinical protocols.
• Conduct nursing assessments and monitor individuals physical and behavioral health needs.
• Educate and empower individuals regarding their physical and mental health.
• Collaborate with interdisciplinary team members to support individualized treatment and recovery goals.
• Maintain accurate and timely clinical documentation.
• Ensure compliance with all clinical regulatory and quality standards.
• Promote overall health wellness and safety for the individuals served.

What You Bring

Were looking for someone who brings the experience skills and commitment needed to succeed in this role.

• REQUIRED: Current unrestricted Virginia Licensed Practical Nurse (LPN) license.
• Experience providing care in a crisis residential or behavioral health setting.
• Strong verbal communication documentation and data-entry skills.
• Proficiency with Microsoft Office applications.
• Knowledge of mental health and intellectual/developmental disability systems of care preferred.
• Experience with crisis prevention and intervention services preferred.
• Valid drivers license and current automobile insurance.
• Commitment to providing compassionate person-centered care.
