MAIN FUNCTION:
A caregiver who assumes responsibility and accountability for a group of patients under the supervision/direction of a registered nurse. Reports to the Director of Department to which assigned.
DUTIES AND RESPONSIBILITIES:
Conduct comprehensive patient assessments by collecting and monitoring physiological psychological cognitive and age-specific data recognizing changes in condition and promptly reporting significant findings to the Registered Nurse (RN).
Implement individualized plans of care under the direction of the RN establishing patient care priorities and delivering safe consistent and evidence-based nursing interventions.
Administer medications and treatments safely according to established policies and procedures including intravenous therapies while monitoring and documenting patient responses.
Demonstrate clinical competency and technical proficiency in nursing skills utilizing sound judgment and maintaining knowledge relevant to the designated patient population.
Promote patient and family education by identifying learning needs reinforcing teaching plans and supporting patients and significant others in achieving optimal health outcomes.
Maintain accurate timely and confidential documentation of assessments interventions patient responses and clinical events in accordance with organizational and regulatory standards.
Ensure patient safety and quality care by adhering to infection prevention practices risk management principles hourly rounding protocols and established nursing policies and procedures.
Respond effectively to emergency situations by following established protocols providing appropriate interventions and documenting all events accurately and thoroughly.
Foster professional communication and collaborative relationships with patients families and interdisciplinary healthcare team members while demonstrating respect accountability and customer-focused care.
Support professional growth and organizational goals through ongoing education competency completion attendance at required training and meetings adherence to legal and ethical standards and participation in unit and nursing division initiatives.

EDUCATION:
Graduate of a Practical Nursing Program registered by the New York State Education or by the licensing authority of another state.
CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS:
Basic Life Support (BLS) required within 90 days of hire date. Advance Cardiac Life Support (ACLS) is required in critical care units.
PHYSICAL DEMANDS:
Appreciable physical effort required. Moving lifting and assisting patients whose size and physical abilities can vary greatly. Some lifting reaching and stooping. Required to be on feet much of the time.
EXPOSURE CATEGORY:
Category I. Tasks that involve exposure to blood and body fluid or tissue.
