Licensed Practical Nurse

• Location: Riedman Administrative Campus / RGMG Care Managers
• Schedule: MonFri 8:00 AM4:30 PM 40 hrs/week No weekends
• Specialty: Adult/Geriatric Care Coordination
• Experience: 1 year Home Health/Mobile Case Management
• Cert: BLS required
• Locals: Accepted; lower rate if <75 miles
• First-Time Travelers: Accepted
• Floating: Rochester General Unity Hospital patient homes & provider offices
• Travel: Reliable transportation required; mobile/home/hospital/office visits
• Role: Transitions of Care / Case Management No hands-on nursing

Responsibilities:

• Work with RN to implement POC
• Telephonic/remote screening follow-up & triage
• Appointment scheduling and barrier management
• Patient education & medication adherence
• Biometric monitoring and EHR documentation
• Home/hospital/provider-office visits as needed
• Connect patients with community resources
• Participate in virtual multidisciplinary care-team rounds
• Manage 2530 active patients / 3040 rotating monthly
• 30-day program with reassessment and transition/closure

Preferred: CHF/COPD complex comorbidities community resource knowledge strong communication/documentation skills.

Key: Primarily Care Coordination/Case Management not bedside nursing. LPNs do not transport patients.
